Capstone has social workers on staff to help the resident and their family members adjust to placement.

Working with the psychosocial needs that may arise during the course of the individual’s stay at the facility, the social worker is not only a source of emotional support, he or she also assists with any necessary adjustments to facility living, mood, financial, and individualized concerns or family requests.

Our social workers regularly meet with families and residents to discuss, evaluate and problem-solve so that the resident can focus on rehabilitation and medical needs with minimal stress and distraction. Residents and families are encouraged to meet with the team members as necessary to resolve any special concerns. These appointments are coordinated through the Social Services office at a mutually convenient time.

The residents of Capstone have a “Resident Council” meeting once per month which gives the residents an opportunity to discuss issues pertinent to the quality of their lives. The Social Workers and the Activities Director assist the Resident Council Representatives maintain an active, involved and united voice that speaks for all our residents.  The Administrator attends the Resident Council, representing the facility’s leadership.   In addition, the facility’s food committee meets in conjunction with the Resident Council to provide an avenue for input on the menu and quality of food.
